To Our Patients and Their Families

入院日Identification of patient

There are many persons with the same surname. In order to avoid mistakes, we identify our patient by both the surname and the first name.

We ask patient to identify him/herself

To avoid any mistake in hearing, we ask our patient to give his/her name to make sure that we have the correct person.

We ask patient to wear an ID wristband

We use the ID wristband to confirm our patient not only when we give treatments such as injection, but also before tests and operations because various hospital staff outside the ward are participating in your treatment.

Making Appointment for Hospitalization

After you have made an appointment for hospitalization, the chief physician of Cardiovascular Medicine Ward will contact you later by telephone or other means regarding the day and time of admission. Please reply to tell us whether you agree or disagree with the admission.
*Car parking for patients being hospitalized is forbidden


Admission Procedures
Before 10 a.m., please come to the “Admission and Discharge Counter” (2nd floor of South Wing, in front of the elevator hall) and complete your admission procedures by submitting the documents listed below. Then please proceed to Heart Center on the 3rd floor.
● Admission application form, health insurance certificate, patient ID card (plastic card)
● Various beneficiary certificates (elderly, disabled, infant, mother and child, atomic bomb survivor, specified diseases, etc.)
● Hospital discharge certificate (if there is a history of hospitalization within the last 3 months)

Payment Procedures

Hospitalization expenses for a given month are totaled and the bill is delivered to your room on the 10th day of the next month. Usually payment can be made from the 12th.

If you have any question regarding the expenses, please ask at the Admission and Discharge Counter (Ext. 5149). Payments after business hours or on Saturday, Sunday and public holidays are handled at the After-hour Reception (entrance of South Wing).

On the day of discharge from hospital, the nurse will contact you after the discharge billing is ready. Please wait in your room. Make your payment at the “Admission and Discharge Counter” (2nd floor of South Wing, in front of the elevator hall) from 8:30 to 17:00, Monday to Friday. Please note that if you are discharged on Saturday, Sunday or public holiday, you may have to settle your payment on another day. The procedures are handled at the After-hour Reception (entrance of South Wing).
After paying the discharge bill, you will be given a receipt. Return to the ward and show it to the nurse. Then, after receiving instructions from the attending physician or nurse, you may return home.


To Persons Who are now Receiving Treatment in Hospitals Overseas

In the case that a Japanese patient or his/her family residing overseas wishes to receive treatment at Kyushu University Hospital, first you should consult with your attending doctor at the country of residence.

Response to any inquiry regarding disease or treatment by telephone or email is prohibited under the Japanese Medical Care Law. Consultation and treatment can only be conducted on an in-hospital basis. We would appreciate your understanding and agreement.

If the attending physician agrees to changing hospital (transfer to Japan for treatment), usually the attending doctor will send a letter or email of referral to the most appropriate department of our hospital.

If the attending physician is not sure which department to send the referral letter, he/she can send it to the address below, and we will transfer it to the department deemed most appropriate.
